The Day Habilitation program is looking for a full-time, 40 hours per week, Paraprofessional to provide instruction and support to individuals with developmental disabilities. The employee works as a member of the Day Habilitation team to provide premier services while assisting program participants with daily living skills including but not limited to dining and personal care needs.
ESSENTIAL FUNCTIONS:
- Follow the daily schedule as written by the Program Director or Coordinator.
- Complete daily data sheets, including documentation for behavior services.
- Implement service plan goals as requested.
- Inform the supervisor or nursing staff of any unusual behavior, sings of illness or injury.
- Ensure individuals are actively engaged in assigned activities.
- Ensure all protocols are followed to include but not limited to: toileting, dining, dietary, PICA (persistent ingestion of nonnutritive substances), behavior guidelines, seizures and safety.
- Ensure positive behavior supports are implemented for all individuals.
- Maintain all program required trainings.
QUALIFICATION
- High school diploma or GED
- One-year experience in a health care, mental health or developmental disability preferred but not required.
- Ability to escort, assist and/or transport program participants.
- Ability to work with mental health and/or developmentally disabled individuals.
- Ability to assist and/or support individuals as needed and in emergencies that require full physical assistance.
- Ability to physically restrain individuals exhibiting unsafe behavior.
- Ability to follow policies, procedures, and directions.
- Ability to work and communicate with others.
- A qualifying criminal background check, driving record and a valid driver’s license required.

Our Day Habilitation program is looking for an employee to serves as an interdisciplinary team member responsible for designing, implementing, and supervising services and assumes primary responsibility for each member on an assigned caseload to coordinate the implementation of the program Service Plan and maintain each member’s record.
ESSENTIAL FUNCTIONS:
- Recommend service plan goals, assist in completing annual severity profiles, and service needs assessments as well as review new potential program individuals.
- Complete daily data sheets, including documentation for behavior services.
- Implement service plan goals, including therapeutic goals.
- Inform the supervisor or nursing staff of any unusual behavior, signs of illness or injury.
- Ensure all schedules are followed and individuals are engaged in activities.
- Ensure all protocols are followed to include but not limited to: toileting, dining, dietary, PICA (persistent ingestion of nonnutritive substances), behavior guidelines, seizure and safety.
- Complete daily communication logs.
- Complete monthly progress reports of service plan goals.
- Maintain all program required trainings.
- Attend interdisciplinary team meetings as needed to participate in curriculum design.
- Implement curriculum in accordance with individual service plans.
- Complete and submit Recipient Severity Profile Forms as required.
- Ensure positive behavior supports are implemented for all individuals.
- Maintain an expertise in the day habilitation field through attendance at meetings, in-services, continuing education or other appropriate educational programs.
QUALIFICATIONS:
- High school diploma or GED required. Two years’ experience working with adults with developmental disabilities or a health care setting.
- Skill in communicating and working both independently and as a team.
- Ability to escort, assist and/or transport program participants.
- Ability to work with mental health and/or developmentally disabled individuals.
- Ability to effectively participate in the design, implementation, and supervision of services.
- Ability to complete and disseminate all required forms and documents.
- Ability to assist and/or support consumers as needed and in emergencies that require full physical assistance.
- A qualifying criminal background check and check and driving record.

People, Incorporated is looking for responsible part time Drivers for a flexible shift, early mornings and late afternoons, to transport program participants to designated locations as scheduled. Experienced Drivers preferred. 7D License preferred but not required. This is a split shift 7am-9am and 2pm-4pm.
ESSENTIAL FUNCTIONS:
- Transport Individuals as scheduled.
- Assist Individuals getting in and out of handicap accessible van or other vehicle.
- Operate assigned vehicle in keeping with all safety policies.
- Perform regular routine inspections of assigned vehicle.
- Inform supervisor when assigned vehicle is in need of service.
- Maintain cleanliness of assigned vehicle.
- Access and read Agency email account and Intranet at least weekly.
QUALIFICATION
- A drug test is required upon hire and random drug testing thereafter.
- Ability to safely operate vehicles including maintaining valid driver’s license and clear driving record.
- Ability to assist individuals getting in and out of handicap accessible or other vehicles.
- Ability to work with both developmentally disabled and non-disabled people.
- Ability to follow directions.
- A qualifying criminal background check and motor vehicle record.

The supported Employment Services Program is looking for a full-time Job coach to provide employment supports to Individuals with developmental disabilities. The Job Coach instructs their assigned Individuals in the work skills and behaviors needed to complete and be successful in their job.
ESSENTIAL FUNCTIONS:
- Instruct assigned individuals in the work skills and behaviors needed to complete and be successful in the job.
- Ensure the quality of work being done meets the standards of the business.
- Maintain accurate daily production records for assigned Individuals.
- Participate in ongoing training programs to improve skills.
- Participate in staff meetings.
- Participate in vocational assessment of assigned Individuals.
- Participate in the development and implementation of training materials.
- Develop and implement job accommodations as needed.
- Participate in job development activities.
- Act as agency representative to contracting businesses.
- Escort, assist and/or transport Individuals as needed.
- Substitute for co-workers vacations, sick and other planned/unplanned absences from work or other necessary staffing but not to exceed 40 hours per week unless otherwise approved by the supervisor.
- Support Individuals in the exploration of landscaping and maintenance
QUALIFICATION
- High School diploma or equivalent.
- Ability to escort, assist and/or transport Individuals.
- Ability to work with developmentally disabled people.
- Ability to communicate and work with others.
- Ability to follow directions.

Our Day Habilitation Program is seeking an LPN to provide instruction and support, as well as oversee all health-care services provided to the program participants. This position will coordinate medical services with each program participant’s primary physician or medical clinic. This position will participate in ongoing interdisciplinary evaluations and review of each program participant’s needs and status.
ESSENTIAL FUNCTIONS:
- Provide direct care and training in relevant areas.
- Coordinate medical services with each program participant’s primary physician or medical clinic.
- Oversee all health-care services provided to the program participants.
- Participate in ongoing interdisciplinary evaluation and review of each program participant’s needs and status.
- Obtain reports and approval of treatment plans from primary physician.
- Document in each program participant’s record any recommendation resulting from the periodic reviews.
- Advise the Program Director and the day-habilitation staff of any medical problems that may hinder a participant’s progress or that may be aggravated by activities at the program.
QUALIFICATIONS:
- Valid LPN license in Massachusetts
- One year of experience in a community health setting
- Skill in communicating and working both independently and within a team.

About Day Services
Our Day Services offer supported employment opportunities, along with therapeutic programs including physical, occupational, speech, art, and music therapies. Among other day activities, groups are available that focus on fitness and exercise; health, diet, and nutrition; and social skills. Clients have the choice of either receiving therapeutic day habilitation services or joining our Employment Services program. The programs are staffed by a dynamic group of well-trained professionals and include registered nurses, occupational therapists, physical therapists, speech therapists, and a number of habilitation support personnel.
Employment services are individualized to assist a person to choose employment outcomes or career development based on his or her preferences, strengths, abilities, and needs. The service will identify sensory, behavioral, and work skill challenges that create barriers to employment. Employment planning uses job exploration by assigning the individual to an Employment Specialist. The Employment Specialists will assist the individual with: interest inventories, situational assessments, job shadowing, resume development, volunteer opportunities, soft skills training, and needs for assistive technology, job accommodations, and job adaptations.
